Advance Care Planning is a process rather than a form, a series of conversations not a one-off event and very much part of everyday life not just something associated in later life.
Clare Fuller teaches members of the public, and health and social care professionals, and hosts a podcast ‘Conversations about Advance Care Planning’.
In England the Universal Principles for Advance Care Planning set out a universal personalised approach to Advance Care Planning and aim to support a consistent approach to “what good looks like” in Advance Care Planning. The work is in response to recommendations in the CQC report Protect, respect, connect - decisions about living and dying well during COVID-19, one of which called for a more consistent approach to Advance Care Planning.
There is no standard Advance Care Plan in England, and forms between areas can differ but ACP is a process rather than a form. This study day will help staff in care homes to have the important conversations leading to the development of an advance care plan

Kate Butcher - Education Lead at OxCERPC, Nurse Specialist
Kate is the Education Lead at OxCERPC and a nurse specialist within the hospice. She brings extensive experience in district nursing and community practice development, working with staff across care homes, primary care, and acute settings. In addition to facilitating learning across a wide range of palliative care topics, Kate has been delivering communication skills training for over 14 years at all levels of competence—foundation, intermediate, and advanced. Kate has an MSc in Clinical Leadership and a Post-graduate Certificate in Psychospiritual Care.
Clare Fuller- Advance Care Planner
Clare is a registered nurse, and a passionate advocate for Advance care Planning. She says “I believe passionately in proactive Advance Care Planning, campaigning to raise public awareness and professional understanding. I have seen first-hand and too often what can happen when vital conversations and plans are left too late and am on a mission to normalise talking about what matters most..”
Clare teaches regularly about ACP, LPAs, ADRTs and end of life care issues, working with individuals and teams to improve care. She also hosts a podcast ‘Conversations about Advance Care Planning’ as well as speaking at conferences. She has worked in a national role for Gold Standards Framework, and works with Care Quality Commission
